need a Kohls rebate form?

I need the rebates forms for the digital picture frames. If anyone has one can you scan it and possibly post it for me PLEASE! I am not driving 80 miles to get them. I will just return them later and get a refund if i cant get the forms. and forget it I guess.

Thanks for the offer. REALLY I don't want anyone making a special trip. I thought someone may have one already. I PM'd Ali if she decides not to go spend her Kohl's cash :) I'll call the store. But I trust her to get them to me before the store :)

I went, looked at the board and asked and they don't have them. The lady said she thinks the pads were empty and therefore removed, as she does remember them from Friday.
