nirvana4all Posted December 2, 2007 Posted December 2, 2007 It's a rebate, so the normal: UPC Address Reciept Proof of purchase Blood sample fingerprint urine sample mothers maden name birth certificate semen sample Certified and notarized copy of authenticity timestamped at post office list of living relatives 3 of your own hairs of at least 4" length

By the way, if this was a purchase you bought in a Staples store, you should be able to submit your rebate online. Here is the link to do that: HERE If you are mailing in your rebate, my suggestion to you is to make a copy of EVERYTHING for yourself, and write the date that you send it out on the form somewhere. BTW, when I say everything, I mean the UPC, the receipt, the rebate form, and any other additional items you send.

I'm new at this rebate stuff since I normally avoid rebates. I started the online rebate thing the same day but I thought i still had to send stuff in is this incorrect?
