This one (same company) has tons of names, and it appears you can pick the one that looks closest to your child and name it ($170-$400)

When I got one when I was little it had a birth certificate with the name and birthdate and had an optional tear off page where you could fill in your own name and mail it in and they would in turn send you a new birth certificate. I did it with my CPK and my Koosa's. When I did the one for my Koosa they sent the BC back with 2 little stickers with the pet name on one and your contact info on the other so you could put them on your pets tags on the collar.

 

I don't know if they still do this but I don't see why they wouldn't.

They don't do this anymore. (Let you sned in for a name change) BUT you can go online with the birth certificate you get in the box and change the name and print one on your home computer. It does not look nearly as nice, but it is an option.

Hey! I was just browsing the CPK site and found out that in fact you still can select the name and birthdate of your kid! YAY!

 

You have to order them through http://www.cabbagepatchkids.com/

From the main page select Play along kids, then browse for the doll that suits your looks best and order you have to specify that you want to change the name and Bday. Prices range from $29.99-$34.99 So if you are looking for custom the extra $10 over store prices shouldn't be an issue. You can customize the Play Along Kids and Magic Colorsilk Kids.(both under the play along link)
