I bought the portable dvd player at Kohl's & the rebate form was on the display. It was the 4x6 paper peel away type. You might be able to go back to your Kohls & get the form.

I ordered that on Friday & already got shipment notification. Also got TARGUS TRG DVD301 portable dvd player carrying case for free(after $10 rebate). Great deal!!!
