This says if you signed up before April 30th it is ok. Not sure what that means, do you think you do not receive the compensation if you fill out the form now???? Has anyone done this? Thanks

it was about working,if you joined earlier you didnt have to be working ,now you do.

My DH was going to do this and he finished the whole 1st section then came the hr by hr summary! He does the same thing all day (CDL driver) and you can not write more than one sentence about it,lol. Anyone else run into this type of problem?

i did i work in a factory doing the same thing for 8 - 10 hours a day i just threw some crap in it for spice!
