email. I think it came directly from Amazon. I cannot be certain though. I had it sent to my business account and that email provider will not display images. There is no way around it with some images. The grayed out areas and the placement of the Amazon code were where they would be if it was a GC that came directly from Amazon.

i have been very busy with surveys the past fews weeks. but right before christmas i was very slow. and i agree with you that we should be paid, but there is nothing that we can do about not being paid. that's all

I know for sure this survey was posted on another message board the day before it was posted with far more than 100 hits at that point.
Also if you do a search right now on google, this survey is now all over the freebie and bargain boards. I hardly doubt that anyone will ever get something from this
