As a token of our appreciation for the time you spend completing our survey, respondents will be randomly chosen to recieve a $10.00 Amazon gift card. Approximate odds of winning are 1:1. Winners will be notified no later than 10/01/2008. Gratuity items will be delivered via Email, and so we are not able to offer shipping to Post Office box addresses. Gratuity items normally take 6 to 8 weeks for delivery. Gift cards are limited to one per person.
To be eligible for a gift card gratuity, you must have completed the survey. You must also be a U.S. Citizen or Permanent Resident with a valid Social Security Number. In accordance with IRS regulations we are required to collect 1099 information (your address and social security number) if the suggested retail value of gratuity items you select exceeds $599 in a given calendar year. Suggested Retail Prices (SRP $) are included along with the list of gratuity items you will be able to choose from.
All information collected on this form will be used solely for the purposes of issuing gratuity to participants in our survey. For our privacy statement, please see: www.microsoft.com/info/privacy
